Mental Health Treatment Plans

What is a GP Mental Health Treatment Plan?

A GP Mental Health Treatment Plan (MHTP) — sometimes referred to as a Mental Health Care Plan, is a structured care document prepared by your GP following a detailed assessment of your mental health needs. It outlines your presenting concerns, your treatment goals, and the strategies and services that will support your care.

Where eligible, a MHTP may entitle you to Medicare-subsidised sessions with a psychologist, mental health social worker, or other allied mental health professional under the Better Access to Mental Health Care initiative.

How Do I Get One?

Book a dedicated mental health appointment, longer than a standard consultation, and let our reception team know the nature of your appointment when booking. Your GP will conduct a thorough assessment, discuss your circumstances, and work with you collaboratively to develop a plan that reflects your goals and preferences.

Important Information About Eligibility

Eligibility for a MHTP and the Medicare benefits available under it are determined by your GP based on your individual clinical presentation and current Medicare guidelines. Not every patient will meet the criteria for a MHTP, your GP will explain what is available to you and discuss alternatives if a formal plan is not appropriate for your situation.

Assessment and Diagnosis Support

Early and accurate assessment is the foundation of effective mental health care. Our GPs conduct thorough mental health assessments that consider your symptoms, history, life circumstances, and any physical health factors that may be contributing to your presentation.

Depending on your situation, assessment may include:

  • A detailed history of your current concerns and how they are affecting your daily life
  • Review of your personal and family mental health history
  • Physical examination and relevant blood tests where indicated, to rule out underlying medical causes
  • Use of validated clinical screening tools appropriate to your presentation
  • Discussion of your current medications and any substances that may be relevant
  • Assessment of risk, where clinically appropriate

Your GP will explain their findings clearly, answer your questions, and involve you fully in decisions about your care.

Ongoing Management and Medication Review

For patients already living with a diagnosed mental health condition, Bopple Surgery GPs provide ongoing care and support, including:

  • Medication review and management — for patients prescribed antidepressants, anxiolytics, or other psychiatric medications; your GP will monitor your response to treatment, manage side effects, and adjust prescriptions as clinically appropriate in line with current guidelines
  • Progress monitoring and care plan reviews — regular review of your MHTP or ongoing care to assess how your treatment is progressing and update goals as needed
  • Coordination with existing treating providers — including psychologists, psychiatrists, and other allied health professionals; your GP will communicate with your broader care team and ensure your care is consistent and well-coordinated
  • Lifestyle and self-management support — evidence-based guidance on sleep, physical activity, nutrition, and other lifestyle factors that support mental health recovery and maintenance

Referral and Coordinated Care

Where your mental health needs extend beyond what can be managed within general practice, our GPs coordinate timely, appropriate referral to specialist and allied health services. This may include:

  • Psychologists — for evidence-based therapies including Cognitive Behavioural Therapy (CBT), trauma-focused therapy, and other modalities
  • Psychiatrists — for complex or treatment-resistant conditions requiring specialist assessment and prescribing
  • Mental health social workers — for practical support with housing, finances, relationships, and social circumstances that affect mental health
  • Alcohol and other drug services — including local and regional support programs
  • Headspace or youth mental health services — for young people requiring dedicated support
  • Perinatal mental health services — for women experiencing mental health concerns during or after pregnancy
  • Community mental health teams — for patients requiring a higher level of ongoing support
